Training Event Registration ~ Winter/Spring 2010

1. Print out the PDF form via the link below and fill in all fields; one form per attendee.

If registering multiple staff, fill out one form per attendee

2. FAX the completed form to Schoolmaster ~ 360.352.0957

If registering for the Early Bird fee, a PO must be faxed to us by the indicated deadline.

3. FAX your purchase order to confirm your registration ~ 360.352.0957.

Attendees wishing to take advantage of the discounted Early Bird registration fee should pre-register ASAP; confirming the registration by faxing a purchase order by the indicated deadline.

Regular and 1-day attendees should fax their pre-registrations as soon as possible and then confirm the registration by faxing a purchase order to our office at least two weeks in advance of the event.
